Referrers

We treat clients aged 18 and over with mild to moderate psychological complaints.

Referring clients

Referrals can be made via Zorgdomein, except when the distance to the practice exceeds 30 kilometres. If Zorgdomein is not available, send the referral letter by secure email toinfo@mulderenvanschie.nl.

What must the referral letter include?

For acceptance by health insurers, the following is required:

• Client's personal details
• Client's phone number and email address
• Referrer's AGB code (and if different: contact person's name, speciality and phone number)
• Suspected DSM-V disorder
• Echelon indication (basis-GGZ or specialist GGZ)
• Whether it concerns a re-registration
